Is there a time frame to bill under a physician, after he passed away?; it was a sudden death, or how come the practice continue billing if he was the provider that co-sign other providers in the practice?
 
Nothing that occurred after the physician's death can be billed under the physician. If it occurred before the date of death, and just hasn't been billed, that can be billed.

Incident-to stops; no one can bill under his name.
