Hello fellow coders, I am new to auditing nephrology charges so I want to be sure I am correct in my review before talking with my provider. Inpatient setting: patient is receiving CRRT. My provider is billing a critical care or inpatient e/m service code for his initial review and decision to start CRRT. Later that same day or night the nurse begins CRRT therapy, the nurse documentation never states provider presence. CRRT takes 24 hours, so on the following day provider's progress note discusses events of CRRT (such as the filter clotted, etc) but provider also never states he specifically saw patient during CRRT. Can the provider bill CPT 90945 if neither he nor the nurse documents his presence during CRRT?
