Is your lab credentialed as a freestanding facility, and do you bill a separate facility claim? If yes, then you'll need to use a facility place of service that corresponds to your type of facility (e.g. 24 - not 22 since that is for a hospital). But if you are not submitting a separate facility claim, then you should use POS 11 so that you'll be reimbursed for both your professional fees and your office technical and overhead costs.
