I work for a physician in opthalmology; in coding dx for rechecks of treatment of plaquenil do you code the V code first or the reason for the treatment. Where could I find written documentation on guidelines for medicare.
 
It depends on if they are currenlty on treatment or complete, if they completed the treatment you use V67.51 if they are still on treatment you use V58.69 as secondary and whatever the disease for the treatment is as primary. It clearly states in the ICD-9 book that V67.51 is a secondary code so i'm not sure if that would be enough documentation for you, hope that helps.
