I work for an OD and I am trying to get riembursment from BCBS for co-managment of Cat Sx and the patients one pair of glasses after Cat Sx... I have billed and been riembursed this for a medicare patient before and I know the codes I need are as follows:

66984-55,LT

What Modifiers do I need for the V codes (if any) since it is not MCE or, do I bill the same way as if it were Medicare????
And, how do I figure out the charges for the 66984 codes???
