1. left anterior tibial and distal popliteal artery endarterectomy.
2. endarterectomy
3. a 6 mL ptfe ringed graft bypass, left femoral artery to anterior tibial vessel of the left lower extremity.

procedure: after general endotracheal anesthesia, the limb was prepped and draped in the usual sterile fashion. Skin incision was made over the left groin, taken down with sharp dissection to the previously placed left limb of the aortobifemoral graft. We then performed a popliteal incision and taken down to the popliteal artery where this was extended down to the anterior tibial. The patient was heparininzed, the clamps were applied, an arteriotomy makde and a #4 Penfield dissector was utilized to form an endarterectomy and establish outflow. The graft was placed through the previously made tunnel and used an end-to-side 6-0 prolene to sew the distal aspect to the juctin of the anterior tibial and distal popliteal artery. Straightened the leg and measured this to the proximal portion of the common femoral artery and limb of the left aortobifemoral graft and sewed it end-to-side 6-0 prolene anastomisis. these both were hemostatic and there was satisfactory pulsation in the graft noted. the wounds were closed with multiple layers of Vicryl suture.
