Procedures:
1. Left heart catheterization
2. Selective Coronary angiography
3. Moderate sedation supervision
4. R innominate artery angiogram

EBL: Less than 10 ml
Complications: None
Lines: None
Specimens: None
Condition: Stable
Anesthesia: Lidocaine 2%
Sedation: Versed and Fentanyl
Moderate Conscious sedation was provided under my direct supervision with the sedation trained nurse using 1 mg of IV Versed
and 25 mcg of IV Fentanyl.
Start time was 1521 and end time was 1559. There were no complications. See hospital trained nurses sedation sheet I signed
and dated for the completed procedure
Access Site: Right radial 6F
Findings:
Left Heart Catheterization:
Left-sided filling pressure: LVEDP _11_ mmHg
LV gram EF: 30%, global hypokinesis
No significant transaortic valve gradient
Coronary angiogram:
Dominance: Right-dominant
LMCA - Large-caliber vessel without obstructive coronary artery disease
