I have providers in a podiatry clinic who are documenting 0 x 0 x 0 sq cm as the pre-debridement measurements when a callus is overlying an ulcer and then they document the size of the underlying ulcer post-debridement. They state that they cannot measure the wound before the debridement, since the callus covers the wound. In a lot of these cases, they are billing 11042, for which pre-debridement measurements are required. My question is.. is it okay for them to use the zero measurements or should they measure the callus for their pre-debridement measurements? Any insight is very much appreciated.
