That would be one level, normally with modifier -50, but that depends on the payer. Some will want you to bill with -LT/-RT and they'll apply MPR 50% reduction to one of the sides. You get paid the same (150%), it's just two different ways of going about it, and usually depends on how the payers pricing software works/how it does the calculations.
