Please help...Can the implant removal code 19330 and the capsulectomy code 19371 be billed together? Are the radical excision and scar revision inclusive to the removal and capsulectomy?

PREOPERATIVE DIAGNOSES: Bilateral breast deformity after breast cancer reconstruction, ruptured right saline breast implant, severe capsule formation and scarring of bilateral breast tissue, severe breast asymmetry. POSTOPERATIVE DIAGNOSES: Bilateral breast deformity after breast cancer reconstruction, ruptured right saline breast implant, severe capsule formation and scarring of bilateral breast tissue, severe breast asymmetry. PROCEDURES PERFORMED: 1. Bilateral breast implant removal. 2. Bilateral total capsulectomies. 3. Radical excision of scar tissue of chest wall in preparation for flap surgery, 125 cm2. 4. Scar revision with complex closure of chest wall, totaling 8.5 cm.
