I have a neurosurgeon who is using duragen during a Craniotomy. Can the physician charge for this? It was done in an inpatient hospital setting and the code we are being given is Q4100.

The physician cannot charge for this - the Duragen (as would be any drug or supply) is inclusive to the facility's case rate. Items such as this should be purchased by the facility and made available to the physician at the time of the procedure. If your physician has incurred a cost for this item, you may wish to invoice the facility directly for reimbursement.
