I am a new coder for psych and need to know how I should code Competency Evaluation to make medical decisions. One patient is on a vent support but sort of alert, family wants to DNR pt doesn't, and the other wants to refuse acute rehab placement.

Any suggestions. Everything I've looked up in the ICD 9 book and on line doesn't give me an answer.

Also how do you determine MDM? (complexity)
