Patient is seen in OFFICE during post op for ileostomy reversal. Dr. dictates combination of chemical debridement and blunt debridement of skin and/ or subcutaneous soft tissue. To me, this is NOT billable as as 11042 with a 78 modifier. Could I please get some other opinions, direction,input, etc.
