I have billed code 11721 with 59 q8
and then i billed 11041 rt or lt depends on person
should both code pay the way i billed this.

THE DX WERE IN ORDER
707.15,110.1 729.5 AND 440.20 OR IF THE ARE DIABETIC IT6 WOULD GO 707.15,110.1
729.5 250.00 AND 250.70
 
Line 1 11041 Dx 707.15 250.xx
Line 2 11721 Q8 59 Dx 110.1 440.20 or 250.xx

I've been doing this for 4 years and get paid by medicare everytime. Make sure you seperate the codes and DX
