Has anyone else seen an inordinate number of denials from NGS Medicare for Routine Nail Care and Palliative Care recently?

Thanks in advance for your time and insight.
 
We also bill NGS for nail care at times. Generally these are denied unless we meet one of the criteria for LCD. Not sure I have seen an increase, but these denials are not uncommon.
