Wiki Poison vs Adverse affect - Would someone please confirm the sequence

Would someone please confirm the sequence order for poisonings and adverse affect with the ICD9 to ICD10. I thought the guidelines stated below but when I took a test, it said I was wrong. ??
ICD9 guidelines say:
For poison, the poisoning code is first 967.0 then the condition 780.09 and then the drug E851.
For the Adverse affect, the reaction condition code is first 780.4 Dizziness and then the E855.0 Adverse Reaction to Dilantin

ICD10 guidelines say:
For poison, the poisoning code is first T42.3X1A then the manifestation/condition R42. and then the abuse if indicated
For the Adverse affect, the reaction condition code is first R42. Dizziness and then the T42.0X5 Adverse Reaction to Dilantin
 
Yes you are correct. Poisoning T codes are listed first with the appropriate A, D, or S as the 7th character. Adverse effect and underdosing T codes are listed secondary with appropriate A, D, or S as the 7th character.
