I got so confused... help code please! Any tips in where I should start?

1.Dorsalis pedis access.
2. Chronic total occlusion of the anterior tibialis artery
3. Successful placement of catheter from the pedal all way up toe the superficial femoral artery
4. CSI artherectomy of the ATA to the popliteal artery
5. Successful drug-eluting stent and balloon angioplasty of the popliteal to ATA artery. Further balloon angioplasty of 2..5 nancross balloon was performed from the mid to the mid distal anterior tibialis with excellent flow and equalization of pressures, baseline cuffs, and dorsalis pedis artery.
6. Wristband placed across foot artery with hemostasis
7. Supervision and Interpretation of above
8. Moderate sedation
9. Ultrasound for vascual access of the pedal artery
11. The ATA was chronic total occlusion with modifier 22 for mental stress

Left groin and left foot were prepped and draped in sterile fashion.
