Wiki port-a-cath coded twice, one with modifier 53?

I am pretty new at gen surg coding and had a question about a chart i am auditing. The patient had a port-a-cath placed. The surgeon attempted access at the cephalic but was unable to pass the guidewire, so he then turned to the subclavian and passed guidewire without difficulty. The coder coded 36561-53 and 36561-59. This doesnt seem right to me. Shouldnt it just be coded as 36561 once only? The dr didnt terminate the procedure which would need to happen to use 53?
