I am auditing a medical record wherein the patient received injections at a clinic but they billed a "facility" fee as well as a provider fee. Is this allowed? The place of service code on the 1500 Claim form is 49. This is the same address and place of service that has previously seen the patient under a POS of 11. I am certain the clinic as a "procedure" room where the provider performs the lumbar and SI injections. Any thoughts? Thanks.
