I have a two part question. We provide primary care services to medicare eligible patients at two assisted living facilities and one independent facility.

In each of the facilities we have a designated space where the patients come to be seen. We rarely see patients in their apartments or home. Should we be billing under place of service 11 for office or 13 for the assisted living?

In the independent living facility we sometimes go into the apartments should those be home visit place of service?
