Is anyone else getting denial for POS is inappropriate with CPT code? Insurance is upholding on appeals as well.

What code and what POS are you billing?

Is a procedure that is considered inpatient only, perhaps?

We currently have them for labs that payer issued notice of not covered in POS 11 rsv machine read and one other I can’t remember right now
