Good morning,
I have submitted a claim to UHC, for a a biopsy of an anorectal wall tumor, anal approach. 45100 this came back rejected because it was done in office. I have looked all over, does this have to be done in a facility?

Is the CPT correct? It wasn't performed with a scope?
