How do we know what icd10 code to use for a routine post op follow up visit? How do we differentiate between z09, z4889, z48.816? Especially for those who have a hysteroscopy d&c done (58558, 0 day global period) and come into the office 2 weeks post surgery for follow up.

Hello, I am not an expert and not sure if my answer is correct ( i don't' think it will create a denial) I would use Z09 when illness is gone and pt has no signs, symptoms. Z09 + history of that illness and z98.890- surgical procedure status. I would use Z48- during post op period when the surgeon is monitoring pt's recovery. Z09-Encounter for follow-up examination after completed treatment for conditions other than malignant neoplasm. Z48.816-

This is a thorny question which we all have to face. You should think of aftercare as the ongoing treatment required during the healing phase after the initial treatment of a disease, while post-operative care specifically addresses therapy needed after surgery for conditions that have resolved. So if you have surgery to remove cancer and it has all been removed, the postop is a follow-up visit. But if you have surgery to do a biopsy to see if there is cancer, and additional visits are required to not only check on the surgical healing, but also evaluate where we go from here, I would call that aftercare.
