We normally see our post op cataract cases the day after surgery and do our billing accordingly for one eye at a time. My question is I have a patient coming in that had surgery on OD and then the next day on OS and then will follow up with us on the third day for Post Op care. Can I bill both post op exams (one for each eye) on the same visit? So basically a 66984 55 RT and a 66984 79 55 LT??
 
Modifier 54 is for the surgeon/surgical care and modifier 55 is for the physician/post op care. We use 66984-55-RT/LT for the first eye and 66984-79-55-RT/LT for the second eye billed. Since the surgeries are different DOS per eye, then you could bill both as long as the dates match the transfer of care dates from the surgeon. We co-manage in our practice with an ophthalmologist who does our cataract extractions.
