A post-op leading to more extensive procedure for positive margins after a lumpectomy is billable with mod 24, but is a post-op visit leading to port placement for chemotherapy billable with mod 24? Since it's the same problem I'm concerned about billing it, but I don't want to miss the H&P/preop charge if I don't have to.

Modifier 24 describes for an unrelated problem within a global period....the post op cannot be billed. For the take back portion, you will bill the code with a mod 58 for these re-excision, that also goes for a port-a-cath placement 36561-m-58. Also see mod 78 for other cases.
