Hi there!
So when they come in for their post-partum visit, you would have your internal "dummy" code for a post-partum. If depression and/or anxiety is assessed without any sort of treatment, then I would include that solely in the PP.
But if there is any counseling, advisement of increased symptoms, and especially any drug prescribing or refilling, then I would definitely code an additional, appropriate leveled E/M with a mod 24.
If provider is merely noting that patient is on existing meds and is doing fine with no concerns whatsoever, then for me personally, I would not additionally add an E/M. That's pretty much stand-alone as an observation.
Good rule of thumb: if the doctor didn't do additional work (which includes additional medical decision making, aka, are they more concerned), then don't charge for it.
