What CPT code(s)/ICD-9 code(s) did you submit?...and where did the services take place?

ok, well this is what the last biller had submitted:

Q9967

Dx 721.3 722.52 722.10 724.2 724.3

I am just supposed to be looking at the status unpaid claims to learn from it and I was just confused by this one...

I believe the issue is HCPCS code Q9967. According to my software, Q9967 is not a match with 64493-64495.
