We have a 94 yr old pt who did not add his daughter to the HIPAA form. The daughter just called and stated she is power of attorney over her dad. Is that sufficient to discuss his medical condition? Dad can always add daughter, but he is from a nursing home and do not know if he is of sound mind to add daughter.

you need a physical notarized copy of the power of attorney in the chart before you can discuss anything with her, then it will be OK.
