a pt came in to get a PPD and she saw the dr.
the dr charged a 99212 and the pt was given the
PPD. the dr didnt see her for anything or discuss anything
except the injection. how would be the best way to send this
to the insurance co?
 
I would not charge the 99212 unless the provider discussed something other than the PPD, and your documentation would have to separately report to meet the 99212 visit. If the only reason the patient came in was for the PPD placement, code only 86580.
 
Agreed. With the V74.1 dx. Also, the reading of the PPD placement is included. So when they come back to have it read, you can't charge again.
