Wiki PPM Check/EKG same day Medicare reduced payment

I noticed that Medicare paid around $2.00 for a pacemaker check and wrote the remainder off with denial B10. The B-10 denial states that the allowed amount was reduced due to the component code was paid. I realize I billed an office visit and EKG same day. I forgot to put the 59 modifier on the EKG showing it was separate . However I would have thought the pacemaker check would have denied bundled--not have the payment reduced?? Please let me know if there is an article explaining this or if you have any input.
