I deal with pathology coding. When coding pathology, you can reference previous specimens to code history. When determining if PQRI has been meet and the cancer check list that provides the 3 measures is not on the current accession, but is on a previous accession, can you use the documentation on the previous accession? The current accession does not make any reference to the previous accession. If you go by the not documented, not done rule where each encounter must stand alone to validate charges, I would so no. But since pathology oftens involved several specimens from the same source, we are having a debate as to whether previous documented PQRI can be used for a current specimen.
