Regarding Measures 110, 111, 114, and 115; please advise when and how to apply these Measures.
We have been instructed to apply the PQRI to a subsequent visit code (i.e. 99212-99215) even if the work was done on admission or discharge day. Some of these PQRI only relate to subsequent visits. For example, Measure 115 doctor provides smoking cessation counseling; as per our instruction if they provide counseling on admission day and nowhere else in the chart, would we still apply the PQRI to the subsequent day 99212-99215 even if they didn't counsel on that date of service.
