The procedures were an exploratory laparotomy, lysis of adhesions and repair paracolostomy hernia...the correct codes were 49460 and 49568 (which I listed) but why wasn't 44005 listed as correct?

That code is specified for when Enterolysis is done as a separate procedure. You wouldn't bill it with other surgical codes.

When you see a CPT code that says (separate procedure), it means this:

This separate procedure, by definition, is usually a component of a more complex service and is not identified separately. When performed alone or with other unrelated procedures it may be reported. If performed alone, list the code.
