We have a patient who presented for an IUD removal and pre-conception counseling. She had complaints of cramping and occasional irregular bleeding.
Physician charted the following: " Plan: IUD removed without complication. Rx for PNV provided. Patient had questions about prior C/S and possibility of TOLAC. We spent 15 minutes reviewing risks associated with VBAC and discussion of labor and delivery issues with TOLAC."
I am unable to locate a diagnosis other than V26.49 (Procreative management; general counseling and advice; other procreative management counseling and advice) for the office visit. Can anyone help me?

Right now I have:

58301 - v25.12
 
In the ICD-9 coding book under counseling-procreative it lists the DX code of V65.49 which is other specified counseling. I have used this code for preconceptual counseling.
