Pt comes in on March 5 for uterine fibroids and provider injects Lupron to help reduce size of fibroids. Pt comes back in on May 2 for followup of Lupron to see if there are significant results. Fibroid has reduced in size so provider notes to schedule pt. for Hysterectomy around June 7. Pt. is scheduled to come back for pre-op examination on June 1. Can he bill for the preop exam on June 1 if he billed the May 2 visit as an E/M with -57 modifier?? Surgery scheduled for June 7.
 
Decision for surgery is already made so standard pre OP E&M is not separately reimbursable. The visit for the post Lupron was the decision for surgery visit.
