If a patient comes to the PCP for a pre-op exam (cxr, ECG, labs) should a regular E/M be used or a consultation E/M?

if the ins carrier still accepts consult codes, use dx series v72.8_ with 99241-99245...make sure you list the md that requests the consult as the referring physician. If the ins does not accept consults, you use a regular E&M

Pre-op exams

If I have a 2year old that has come in for a pre-op physical for dental work for dental caries. How would I code it? Would I use V72.84 and 521.00(dental caries) then use an E/M preventive with a E/M plus modifier 25. HELP!!!!
