Dictation documents visit was pre-op. Surgery was a week away. How should this be billed?
 
Short answer--If the pre-op was routine (patient was otherwise healthy except for condition necessitating surgery), then the visit is included in the surgical global days. If the patient has other existing conditions that need to be evaluated before patient can be cleared for surgery, then you may be able to code & bill the E&M.
