Our doc likes to see patients in the office prior to doing colonoscopies. No problems with this visit when they're diagnostic, but we have a lot of issues when patients are seen prior to a screening. Several insurances that we deal with deny the screening diagnosis with a regular e/m code, but the notes do not substantiate an actual preventative visit. Does anyone else have this problem? Thanks!
