Wiki Pre-surgical clearance evaluations

Good Morning,

I am new to Behavioral Health coding and I need some help with this. What CPT codes would you use for these types of clearance. My provider is having patients referred for clearance by Pain Management providers before placement of a Neurostimulator. Some of the information I am finding is to use 90801 and others are stating to use 99204/99214 depending on whether the patient is new or established.

MH can use both set of codes (not at same time) depending on the documentation.

Can an lcsw and an lmft bill for these codes as well and what kind of documentation is needed?
