Can anybody tell whether it is mandatory to code 285.9 with 648.2x

Category 648 has a convention which states use additional code to specify the condition you need the 648.2- first listed ( fifth character mandatory)
648.2 and 285.9

Additional codes from other chapters can be used in conjunction with chapter 11 codes to further specify conditions if necessary. They are listed as secondary diagnoses. The 648.2X code is sequenced first, followed by the code for anemia, 285.9. Code 285.9 is used to show the type of anemia. See the note below 648: Use additional code to identify the condition.
But in some codes like 643 series does not having this note. Also those codes giving the full description for condition.
Excludes Note indicates that an additional code may be required to fully explain the condition.
The condition in the exclusion note (648.2) is not included in the code under review
If the condition specified in the exclusion note is present, the additional code should be assigned along 280-289 series.
