When a pregnant patient comes to our MFM department for ultrasound does the documentation have to state its a high risk pregnancy? because I see alot of indication for the tests that just say: IVF pregnancy,AMA, hx of fetal demise, and hx of miscarriage and some of those code to the V23 category
