Hello I can't find a guideline or coding clinic pertaining to when the provider uses the term approximately 8 weeks pregnant or the same verbiage but approx 8 weeks pregnant by LMP. I looked at the uncertain guideline and the word approximately isn't listed there. Would you code either of these to 8 weeks or 1st trimester if given or would you go with unknown weeks and unknow gestation. If there is a guideline or CC can someone point me to it?

The guideline does state "or other similar terms indicating uncertainty." IMHO approximately is a term of uncertainty. I think I would still use first trimester but unknown weeks.
