hello, does anyone know how to use these codes, when to use them, the rules regarding them etc. h1000, h1001, h1002, h1003, h1004, h1005. I was informed it's up to payer policy but for instance i have attached the united health commercial policy which is typically the strictest for commercial insurance anyway for us in our state and there's no mention of them. I've seen some states require a prenatal risk assessment form to be filled out and i saw some of the codes are continued care. do you charge anything for these or are they in replace of 0500f and 0502f for instance.

If anyone has any information that would be great i will attach them and the commercial policy

If your payer requires them, this information should be included in their provider manuals. Or if you don't have access to them at your first denial of a claim for not having them (and then make a note of it for future reference). This are not codes for payment, they are codes for clinical excellence information which may be required by many Medicaid programs, but rarely required by commercial payers unless you have signed up the quality of care bonuses.
