What ICD codes are appropriate for a preop inpatient consultation for hip dislocations, other problems evaluated being CAD, HTN, chronic smoker?

As per official guidelines, V72.83 should be the PDX followed by reason for the surgery and the other chronic conditions. But if you bill for medicare, the PDX would be either reason for the surgery or the chronic conditions being addressed and V72.83 as additional diagnosis.
