Is provider stating “stable on Coumadin” sufficient to capture prescription drug management.
 
I would say no.
Provider is stating that the patient is stable on the medication, but in order to count Rx management, the provider must be prescribing a new Rx, stopping an Rx (due to side effects), changing an Rx (due to cost or side effects) or renewing an Rx.

Personally, I ask my providers to document the dose, but there are differing views on that.

I agree that "stable on coumadin" does not count for prescription drug management. If the statement said "stable on coumadin; continue 3mg QD." that I would count. Even "stable on coumadin; continue current dosage" would be the bare minimum (and not suggested). If they are properly using the EMR, the dosage will already be there. There needs to be a decision to change, continue, or discontinue the prescription. Stating it is stable without any further instructions do not demonstrate a decision in my opinion.
