Would this count as Prescription Management in an inpatient setting? I was always told it the provider should have the dosage amount. I wasn't sure and have been getting conflicting information if I can use the medicine reconciliation list.

3. When can prescription drug management be credited in the medical decision-making? (From Novitas- PA)
Credit is given for prescription drug management in the risk of complications and/or morbidity or mortality of patient management when documentation indicates medical management of the prescription drug by the physician who is rendering the service. Medical management includes a new drug being prescribed, a change to an existing prescription or simply refilling a current medication. The drug and dosage should be documented as well as the drug management.

In the history portion of the report it gives a prescription Medication reconciliation list of the hospital meds with dosage.
Notes state:
Patient was on PO Keflex prior to discharge on 09/27
Readmitted on 10/14
Okay to continue IV Zosyn while in hospital.
