Good morning,
When a teaching physician makes a change to a residents inpatient visit note, does that then make the note the teaching physicians documentation and so an attestation stating presence is not needed?
I get really confused by this. I can see if they make a change to the HPI or Exam, but the assessment - I think that they can make those changes just by discussing the case with the resident and not necessarily be present during the visit.

Any help/guidance is appreciated.
