Can we bill a procedure and prev exam on the same day?
Also, can we bill for an e/m code (ex.99213) and procedure on the same day? Many of our patients come in w/ chronic problems that are reviewed at the time of the procedure.

Yes, as long as the documentation supports both services.

You don't have to have separate notes but there must be enough documentation that each charge can stand alone.
