Does anyone have any good documentation or resources for "training" providers on the difference between a routine preventive exam and a diagnostic E/M visit. I have a couple of providers that always want to use the chronic diagnoses as the main reason for the visit and code it as a 99214. Thanks in advance for any help.
 
Kathy, I would quote them exactly from the CPT book, as it describes a preventive medicine evaluation. Example: "It reflects an age and gender appropriate history/exam and is NOT synonymous with the "comprehensive" examination required in E/M codes 99201-99350. 99381-99397 includes counseling/anticipatory guidance/risk factor reduction interventions which are provided at the time of the initial or periodic comprehensive preventive medicine examination."
