I have a question for all: Can you put a preventative well check visit and a office visit with modifier 25 on the same ticket?
 
Yes of course you can. As long as the problem/illness found is separate and identifiable outside the scope of the preventive, you may code it. You should look for a separate HPI and the exam elements should be very specific to the problem at hand. Documentation is key and the illness canNOT be trivial. If the problem is not worked up and is simply just mentioned in the record, it does not warrant both codes. Hope this helps!
